    Hydroxychloroquine is a derivative of chloroquine that has both antimalarial and antiinflammatory activities and is now most often used as an antirheumatologic agent in systemic lupus erythematosis and rheumatoid arthritis. Hydroxychloroquine therapy has not been associated with liver function abnormalities and is an extremely rare cause of clinically apparent acute liver injury. The recommended hydroxychloroquine dosage for rheumatoid arthritis is 400 mg to 600 mg a day by mouth as a single dose or divided into two separate doses. However, once a good response is being maintained with hydroxychloroquine, doses may be reduced to 200 mg to 400 mg per day for maintenance. The recommended adult dose for rheumatoid arthritis is 400-600 mg daily for 4-12 weeks followed by 200-400 mg daily. Systemic lupus erythematosus is treated with 400 mg once or twice daily for several weeks then 200-400 mg daily. Hydroxychloroquine should be taken with food or milk in order to reduce stomach upset.
