For the treatment of RA, hydroxychloroquine is typically taken by mouth once daily at 200-400 mg, although it can be taken at a higher dose. A loading dose of 1,200 mg daily, given for 6 weeks may decrease the time to clinical response. However, experts debate that this higher dosage may increase the risk of toxicity. A controlled, double blind, parallel group, long term study of hydroxychloroquine sulphate in the treatment of rheumatoid arthritis, comparing daily doses of 200 mg and 400 mg, is described. The trial involved 54 patients with moderate disease activity who had not previously received antimalarial drugs. The mean AUC of methotrexate was increased 52% and the mean Cmax was reduced 17% when a single dose of methotrexate was given with a dose of hydroxychloroquine 200 mg oral. Close monitoring for evidence of methotrexate toxicity should be done in patients receiving this combination, especially in those with reduced renal function.

Taking hydroxychloroquine long-term or at high doses may cause irreversible damage to the retina of your eye. Hydroxychloroquine is also an antirheumatic medicine and is used to treat symptoms of rheumatoid arthritis and discoid or systemic lupus erythematosus.

Nov 09, 2018 Hydroxychloroquine is also used to treat symptoms of rheumatoid arthritis and discoid or systemic lupus erythematosus. Hydroxychloroquine may also be used for purposes not listed in this medication. Hydroxychloroquine is generally prescribed at a daily dose of 6.5 milligrams or less per kilogram of body weight. Using this formula, the dosage for a 150-pound person would be 443 mg/day, as one kilogram equals 2.2 pounds. Because HCQ is formulated as a 200 mg tablet, many people taking it for lupus will take two pills per day. Tablets, 2.5 mg; vials, 5 and 50 mg for parenteral use. Dosage. For RA and for psoriatic arthritis, 7.5 to 10 mg weekly may be used initially, administered orally or parenterally, and increased as clinically indicated. The optimal dose for RA may fall in the range of 15 to 25 mg weekly.

G6PD and Malaria – There is also some evidence that quinine can cause haemolysis in patients with G6PD deficiency; such patients may also have increased blood concentrations of mefloquine when taken concurrently with primaquine. As such, combinations of quinine or mefloquine with primaquine in G6PD-deficient patients is not recommended.
